Please complete the steps listed below if you would like to receive a refund for your purchase  of unexpired Afrin^® Original  Nasal Spray 6 mL (0.2 oz) bottles with lot numbers 230361, 240822, 241198, 250066, 250152, 250646, and 250831 impacted by our April 23, 2026 voluntary recall.

In order to complete this form, you will need to provide:

Name, Address and Email
Product Name and Quantity of items purchased
Proof of purchase (photo(s) of the item(s) showing the Lot Number(s) printed on the back of the bottle) and of the disposal of the product
